As he was asleep, he dreamt about Megumi entering Toru house to find him. The Megumi in the dream was jealous of him liking Toru more then her, hence she bit him with her fangs

Toru died in episode 5.
Apparently the Megumi that Natsuno dreamt about was REAL and towards the end, Masao was attacked by what was suppose to be a vampire. As for what happened next, we will have to wait for the next episode.
Kateikyoushi Hitman Reborn was really intense recently with G's Archery on episode 195 and Lampo's Shield on episode 196.
Similarly, Fairy Tail is getting intense too with the nearing of the end of the Tower of Heaven Arc.
